We use the most current equipment for digital x-rays and digital photography, have flexible appointment scheduling, and a professional and caring staff to assist you during treatment. When you leave our office, we want you to love your smile and feel even better about yourself.

Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.

Insurance
If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. Remember that insurance policies vary, and we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims.
We are a preferred provider for Delta Dental, BlueCross Blue Shield, Cigna and United Concordia. To help us with your insurance filing, please complete the insurance questions on our questionnaire.
Please remember that orthodontic insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges
Financial Information
Orthodontic treatment is an excellent investment in the overall dental, medical and psychological well being of children and adults. If treatment is indicated, Diagnostic Records (models of the teeth, photographs, and special jaw X-Rays) are necessary to finalize an orthodontic treatment plan. The Records are reviewed and treatment options are discussed. The Records/Case workup fee is $390. Orthodontic treatment may be deferred for up to 12 months without incurring additional fees.
Orthodontic treatment fees are generally all inclusive. Early, limited (or Phase 1) orthodontic treatment ranges from $1500 to $3700. More comprehensive (or Phase II) orthodontic treatment ranges from $5300 to $7900.
Financial considerations should not be an obstacle to obtaining this important health service. We are sensitive to the fact that different people have different needs in fulfilling their financial obligations, therefore we provide several payment options:
Option A: No Initial Payment Plan (Springstone, 1-800-630-1663, springstoneplan.com)

- One low monthly payment for multiple family members

- Prepayments can be made anytime without penalty

Option B: Office Payment Plan
- An initial payment is due when treatment begins
- The balance may be paid through consecutive monthly/quarterly payments
- Prepayments can be made anytime without penalty
- No interest on accounts kept current
Option C: Payment in Full
- A bookkeeping courtesy of 3-8% is given (depending of duration of active treatment).
Option D: We also accept Visa, Discover and Mastercard.
Insurance Information
If you are eligible for orthodontic benefits under your plan, we will be happy to file your insurance claim for you so that you may be reimbursed for your payments to this office. We participate with the following insurance programs: Altus Dental, Blue Cross/Blue Shield, Cigna, DeCare Dental, Delta Dental and United Concordia.
Pre-tax/Flex-Spending Plan Information
Please verify with your Employer or Third-Party Administrator the details of your Pre-Tax Plan. Each plan has unique requirements. Financial arrangements with our office are independent of any such plan; and the beneficiary, not our office, is responsible for compliance.
